For submission to the Graduation area (Woody A-117):
- Graduation Application for current undergraduate students is now an online application available within SalukiNet Self-Service (for other student situations, please go here)
- for undergraduate students who need to make a change to their already-submitted Graduation Application, please submit changes in writing to us at Woody A-117, or via fax at 618-453-2931, or via e-mail at regstrar@siu.edu (and be sure to include your name and Dawg Tag number)
- Request for Reprint of Diploma
For submission to the Registration area (Woody A-115):
- Request for Refund of the Undergraduate Student-To-Student Grant Fee
- Current Term Enrollment Verification Request
- you should see your academic advisor to complete forms related to your registration (e.g., late course adds or drops, change of major, adjustment of maximum hours, early reinstatement from suspension)
