Students should see the advisement unit within their college if they are interested in changing majors, adding a concentration to their major, adding a minor, etc. Undergraduate students may need to satisfy a certain minimum GPA in order for such changes to be permitted (see Chapter 2 of the Undergraduate Catalog [Advisement, Registration, Withdrawal section, Changing Majors subsection] for details).
Formal paperwork requesting curricular changes for a term must be received by the Registrar's Office (or the Graduate School in the case of graduate students) by Week 2 of the term; otherwise, the change will be put into effect for the following term. Students who are approaching graduation should verify that their academic program officially on record with the University is up-to-date before they submit their graduation application.
Effective March 2014, most undergraduate students who have not yet earned a Bachelor's degree, and who have earned more than 45 total credit hours (including more than 26 SIU credit hours), will need to formally declare a major in a degree-granting program if they have not already done so. Address and Phone Changes
Students may submit address and phone changes within SalukiNet, selecting the MyRecords tab and entering SalukiNet Self-Service. You may provide separate permanent (home), mailing (local), and billing addresses. Information about how addresses are used is available here.
These address/phone changes will affect only your SIU student records. If you are also an employee, graduate assistant, or student worker, you will need to contact Human Resources and/or Payroll to update your address/phone within their system.
Emergency Contact Information Changes
Students may supply and update emergency contact information within SalukiNet, selecting the MyRecords tab and entering SalukiNet Self-Service. In the event of an emergency directly impacting the student, appropriate SIUC personnel will be able to use this information to quickly contact family (or other persons noted in the emergency contact information) with information about the student.
Name changes cannot be done within SalukiNet Self-Service. Instead, please present the completed Name Change Request Form to the Registrar’s Office in the Student Services Building, room 0251, along with official identifying documentation. Acceptable forms of ID include a state-issued picture ID and one of the following:
- Marriage Certificate
- Divorce Decree
- Court Order
- Birth Certificate
- Passport (for international students)
If you cannot come to our office in person, you may submit the completed form and copies of the official documentation by mail to the Registrar's Office, Student Services Building, room 0251, Mail Code 4701, 1263 Lincoln Drive, Carbondale, IL 62901. Your Social Security Number (SSN) is required if you are applying for financial aid, especially for Title IV loans and grants tracked through the National Student Loan Data System (NSLDS). Your SSN is also required if you wish to receive a Form 1098-T "Tuition Payments Statement" that you can use to claim certain education-related tax credits (see IRS Publication 970) that will reduce your annual federal income tax.
If you need to verify and/or correct the SSN that is on file with the University, you should check with the Registrar's Office, the Financial Aid Office, or the Bursar's Office, preferably in person on the second floor of the Student Services Building. Having a copy of your Social Security card to show to an appropriate staff member will be required if there is a problem with the SSN currently on file.
The Bursar's Office web site has a W-9S form that can be completed, signed, and mailed in if you are unable to come to the Student Services Building. SSN changes cannot be done within SalukiNet Self-Service.
