Changing Your Personal Information
Update your personal information, including your name and mailing address, in our system.
Name Change
Legal Name (Students)
Your legal name is required to be used by the University in a number of areas necessitated by University business or legal requirement (such as paychecks, financial aid, the official transcript, etc.).
Making a change to your legal name can be done by completing the Name Change Request Form. Submit the completed form to the Registrar's Office in the Student Services Building, room 0251, along with official identifying documentation. Acceptable forms of ID include a state-issued picture ID and one of the following:
- Marriage Certificate
- Divorce Decree
- Court Order
- Birth Certificate
- Passport (for international students)
If you cannot come to our office in person, you may submit the completed form and copies of the official documentation by mail to the following:

You may also fax the request to (618) 453-2915. If you have any questions about this, you may call (618) 453-2993 during regular business hours (Monday through Friday, 8:00 AM to 4:30 PM CT).
This legal name change will affect only your SIU student records. If you are also an employee, graduate assistant, or student worker, you will need to contact Human Resources and/or Payroll to update your legal name within their system.

Contact Human Resources and/or Payroll to make similar updates to your legal name within their system.
Preferred Name
Enter or update your preferred first name that you would like used by the campus community, instead of your legal first name.
You may identify a preferred name by logging into SalukiNet, searching for the “preferred name” task, and completing the process. Please allow up to five business days for the change to be reflected.
Update Additional Information
It's important to make sure your information is accurate and up-to-date so you receive communication from the University.
Address / Phone
You can change your address and phone number online at Salukinet.
Employees, graduate assistants, or student workers, will need to submit a change of address form with Human Resources and/or Payroll to update your address/phone.
Emergency Contact
You can designate and update emergency contact information online at Salukinet.
Social Security Number
SSN changes cannot be done within SalukiNet Self-Service or via a phone call.
If you need to verify and/or correct the SSN that is already on file with the University, you may do so in person or by mailing the required documents.
In Person
Visit the Registrar's Office on the second floor of the Student Services Building. You will need to bring a copy of your Social Security card to show to an appropriate staff member. Alternatively, you can visit the Financial Aid Office or the Bursar's Office, both located on the second floor of the Student Services Building.
Submit by Mail
If you are unable to come to the Student Services Building to submit and/or correct your SSN, you will need to provide proper proof of your SSN via US mail.
- Do not send SSN documentation via email since email is normally an insecure method of transport.
- Send copies of the front and back of your Social Security card via US mail to the postal address noted under the Contact Us section on the left side of this web page.
Another option is to complete, sign, and submit the W-9S form available on the Bursar's Office website. Mail the completed form to the postal address noted on this web page. However, the preferred documentation to mail is a copy of your Social Security card.
